Please contact our office during regular business hours to schedule an appointment. If you are unable to keep your appointment, we ask that you contact our office at least 24 hours in advance so that we may use that time for other patients.
At the time of your appointment, bring your prescription medications in their current containers, insurance forms and identification cards.
You may contact our office during office hours to request a prescription refill. Please allow at least 24-hours for the request. You may also contact your pharmacy to request a refill. Medication refill requests are sent electronically from the pharmacy to our electronic health record system. Contact your pharmacy prior to picking up the medication to verify that the request was approved by the physician. For safety reasons, we do not refill controlled drug prescriptions after hours.
